It's made from a Marvel legends Blob body and upper torso, DCUC Kalibak arms and pegs, Mr. Potato Head hands, and a Pete Pez Dispenser head. Plus lots and lots of additional sculpting.
I'm not much into KH figures, but from your parts list you really went to work on this figure in terms of sculpting and paint with some excellent results.
