The arm bracers is a personal touch, because I want to give him some "maniac surgeon warrior" look, but you are right originally he looks like a "maniac surgeon from the worst nightmare" with a "little" knife and surgery gloves!!!
Awesome movie-style Pyramid Head custom, other than the arm bracers which he doesn't wear. The paint is great and his skirt/apron very realistic in texture. That said, I never liked the movie version myself, as the helmet is too inaccurate to the source material (too detailed and the Japanese name for the creature is 'Akai San Koku Tou' -- literally 'Red Pyramid Thing', so choosing not to give it a red head, like in the Silent Hill 2 game source material, is just fundamentally wrong, kind of like making Green Arrow purple).
